- Enter Provider Code: 2104 followed by # key
- After Dr. Mann is verified press 1
- Enter your Social Security Number followed by # key
- If you have had labs or radiology procedures performed and have not received the results after 3 weeks please contact us.

How do I get my prescription refills?
- If you are in need of refills please first contact your pharmacy to determine if you have remaining refills. If you do not, please ask your pharmacy to fax a request to our office.
