- Enter Patient ID: Social Security #
- If you have had labs or radiology procedures performed and have not received the results after 10 days please contact us.

How do I get my prescription refills?
- If you are in need of refills please first contact your pharmacy to determine if you have remaining refills. If you do not, please ask your pharmacy to fax a request to our office. If you have any remaining medication refill question please contact us at 830-980-9686.

- We accept most insurance plans. Please call your insurance company and ask for a list of providers in your area or visit their website for this information.

How do I reach the doctor for urgent matters after-hours?
